DA Form 5501, Body Fat Content Worksheet (Female) , is a form used for recording the body fat content of female soldiers enrolled in the Army Weight Control Program. The form is meant for detailing their body fat percentage and tracking the weight loss process. A loss of 3 to 8 pounds of weight or 1 percent of body fat is considered to be a safe and attainable goal for soldiers to reach within a month.
The latest version of the form - often incorrectly referred to as DD Form 5501 - was released by the U.S. Department of the Army (DA) on May 1, 2013 . An up-to-date DA Form 5501 fillable version is available for digital filing and download below or can be found through the Army Publishing Directorate website.
The female body fat content worksheet was previously called DA Form 5501-R. The form is currently used during routine weigh-ins performed once every 6 months and as a monthly assessment as a part of the Weight Control Program. A separate DA Form 5500, Body Fat Content Worksheet (Male) exists for male participants of the program.
