What Is DA Form 5501?

DA Form 5501, Body Fat Content Worksheet (Female) , is a form used for recording the body fat content of female soldiers enrolled in the Army Weight Control Program. The form is meant for detailing their body fat percentage and tracking the weight loss process. A loss of 3 to 8 pounds of weight or 1 percent of body fat is considered to be a safe and attainable goal for soldiers to reach within a month.

The latest version of the form - often incorrectly referred to as DD Form 5501 - was released by the U.S. Department of the Army (DA) on June 1, 2023 . An up-to-date DA Form 5501 fillable version is available for digital filing and download below or can be found through the Army Publishing Directorate website.

The female body fat content worksheet was previously called DA Form 5501-R. The form is currently used during routine weigh-ins performed once every 6 months and as a monthly assessment as a part of the Weight Control Program. A separate DA Form 5500, Body Fat Content Worksheet (Male) exists for male participants of the program.

DA Form 5501 Instructions

An overview of the DA Form 5501 and additional information can be found in the Army Regulation 600-9, The Army Body Composition Program. Filing instructions are as follows:

  1. The top lines of the form require the service member's personal information: name, age, and rank. Weight and height measurements also should be indicated here.
  2. Boxes 1 through 3 are for recording the neck circumference (measured below the larynx), abdomen measurements (taken around the navel), and hips measurements at the point where buttocks protrude backward the most.
  3. Each measurement should be taken three times and each try should be recorded. The average result for each measurement is calculated and recorded in the last column.
  4. Section 4 is for measuring and recording the average waist and hips circumference.
  5. Part G of Section 4 is for entering the body fat value that intercepts with the circumference value and the soldier's height. That number is the body fat percentage value of the evaluated soldier. All values can be found in Figure B-2 of the AR 600-9, (Percent Fat Estimation for Women).
  6. The form should be dated and signed by the personnel performing the measurements for it to be complete.
