Provide the following information in Form CMS 100:

- Indicate two work county preferences;
- Check the appropriate box to describe your availability - permanent employment, temporary employment, or both;
- If you are eligible, claim veterans' preference and attach documents that prove it;
- Sign and date the Illinois CMS 100 form;
- If you have ever been fired from a job or are currently in default on the repayment of any state educational loan, answer "yes" and explain the circumstances;

- Indicate undergraduate and graduate hours for the courses you have completed;
- Record your work history - current and past employer details, your position titles, dates of employment, the average number of hours per week, and the reason for leaving. Describe the duties you have performed, and if you had supervisory responsibility, enter the number of employees you have supervised in various fields. Note that you are not required to disclose your prior salary or wage history;
- Add your email address for communication about opportunities;
- State your ethnicity. If you have a disability, check the appropriate box.
