  12. Indicate undergraduate and graduate hours for the courses you have completed;
  13. Record your work history - current and past employer details, your position titles, dates of employment, the average number of hours per week, and the reason for leaving. Describe the duties you have performed, and if you had supervisory responsibility, enter the number of employees you have supervised in various fields. Note that you are not required to disclose your prior salary or wage history;
  14. Add your email address for communication about opportunities;
  15. State your ethnicity. If you have a disability, check the appropriate box.
