Form VTR-34, Application for a Certified Copy of Title , is a form that should be completed by the owner of a vehicle to replace a Texas Certificate of Title in case the original document is lost, destroyed, or mutilated. If there is a lien on a vehicle, a VTR-34 Form must be filled in by the lienholder. If an applicant is not a lienholder, they should provide an original release of lien. A Certified Copy of Texas Title can be requested to replace the original certificate or any previously issued copy of it. An applicant who needs to apply for a Certified Copy of Title should wait 30 days after the last title was issued.
If the owner or lienholder of a vehicle needs to obtain the information contained in the motor vehicle record, Form VTR-34 is not suitable for this purpose. They should complete Form VTR-275, Request for Texas Motor Vehicle Information in this case. A record request can be submitted to obtain a title history, or current or expired title and registration verification of a vehicle record.

The owner or lienholder should attach to the completed VTR-34 Form a copy of their government-issued photo ID. If there are several owners of a vehicle, each owner should sign the Application for a Certified Copy of Title and provide a copy of their government-issued photo ID. The filer has to attach to the form a document confirming payment of the applicable fee as well. Its amount depends on the method of applying. These fees are non-refundable.
An applicant should submit the required documents by mail or in-person to the Texas Department of Motor Vehicles (TxDMV).
