Form VTR-34, Application for a Certified Copy of Title , is a form that should be completed by the owner of a vehicle to replace a Texas Certificate of Title in case the original document is lost, destroyed, or mutilated. If there is a lien on a vehicle, a VTR-34 Form must be filled in by the lienholder. If an applicant is not a lienholder, they should provide an original release of lien. A Certified Copy of Texas Title can be requested to replace the original certificate or any previously issued copy of it.
