SBA Form 3300, Award Nominee Background Form is a document used to provide general information about each person nominated for a Small Business Administration (SBA) Award. The document is used as a part of the nomination package used for electing nominees. The list of additional forms required for a specific award for the current year can be found in the annually-updated National Small Business Week Award Nominations Guidelines.
The SBA released the latest version of the form on September 1, 2017 , with all previous editions obsolete. An up-to-date SBA Form 3300 fillable version is available for digital filing and download below or can be found through the SBA website. The form must be completed by each individual, each team member (for team nominees) or by the official representative of a small business nominated for an award.
SBA Form 3300, Award Nomination Form requires the same data as the Award Nominee Background Form and is used for providing background information on the award nominees.
Any individual that owns or operates a U.S.-based small business can take part in the competition for one of the SBA's annual awards. Any third party may nominate a team, a single business owner or an entire business for one of the awards listed above. Teams and business owners may also self-nominate.

Each award has several levels with the SBA District Office Level being the lowest and the National Level being the highest. The winners of the lower-level nominations are nominated for higher levels of competition.
All award applications include the background form and the application form specific to the desired award. There are fourteen different application forms in total:
