  2. Confirm the award for the individual, whether an AF member or a civilian employee, who has achieved success and honored the public image of the entire military.

  4. Grant AF members and their families benefits and healthcare they require during the relocation and in emergency situations.
  5. Prepare for travel - whether by public transportation or by your own vehicle - and determine possible risks of the trip with the person whose duty it is to supervise the travel arrangements.

Popular U.S. Air Force (AF) Forms

The AF Forms and Publications below are among the most widely used by AF members, supervisors and subordinates, trainers and trainees. Every Air Force Form is designed for a particular purpose - learn how to properly complete the official documentation and where to submit the forms to obtain the result you are looking for.

  • AF Form 422, Notification of Air Force Member's Qualification Status. Prepared for all AF personnel, this document outlines what each individual is qualified to do after their medical assessment. With the help of this form, commanders and system specialists can manage their subordinates and make informed choices using fitness tests and limitations of airmen presented in simple terms;
  • AF Form 428, Privacy Statement, is prepared by supervisors who can order overtime for their subordinates if the mission requires it and the work cannot be accomplished within the usual 40 hours per week, track excess hours of work, and compensate employees for their overtime efforts, including work during the holiday period;
  • AF Form 707, Officer Performance Report (LT Thru COL). Filled out for officers from the rank of the lieutenant to the colonel, this document records their performance history and allows for the promotion recommendation. It is also designed to carry out other management decisions such as early retirement or new assignment for selected individuals;
  • AF Form 910, Enlisted Performance Report (AB Thru TSGT), is used to record the performance of AF members from the rank of the airman basic to the technical sergeant. The AF examines how effectively every member performs their duties, sets expectations for all personnel, and recommends certain individuals for promotion;
  • AF Form 911, Enlisted Performance Report (MSGT Thru SMSGT). Similar to AF Form 910, this statement provides the overall assessment of the AF member's performance and estimates the possibility of promotion. Complete this document for personnel between the rank of the master sergeant to the senior master sergeant;
  • AF Form 931, Airman Comprehensive Assessment (ACA) Worksheet (AB Thru TSGT), is required to assess the knowledge and qualifications of the AF member after you conduct an interview with them. It will confirm their readiness for deployment and track their development - a worksheet provides the airman with feedback and lets them improve their performance;
  • AF Form 1206, Nomination for Award. The AF has issued this form to recognize the outstanding achievements of its military and civilian personnel and reward them for their behavior. Specify the award that should be presented and list the accomplishments of the person who will receive it;
  • AF Form 4380, Air Force Special Needs Screener. If an AF member has a relative with special needs, they have to provide information about their family member to coordinate the potential relocation with due regard to this person's health condition and let the AF decide whether the military family member requires any specialized care and benefits;
  • AF Form 4392, Predeparture Safety Briefing. When supervisors and commanders plan a trip, they need to thoroughly plan it and comply with existing safety protocols - find out and minimize risks using the proposed itinerary, especially if the AF member plans to travel using their private vehicle to go on leave, change their station, or temporarily relocate to complete a duty assignment.
