DD Form 2958, Service Member Career Readiness Standards/Individual Transition Plan Checklist is an ITP checklist called the Capstone. The Capstone is filled out by the service member during the Transition Assistance Program (TAP) briefing to assure that they were actually provided the training.
This form - often incorrectly referred to as DA form 2958 - was released by the Department of Defense (DoD) on August 1, 2013 . A DD Form 2958 fillable copy is available for download below. Service members will also receive a printed version of the checklist during their transition program training.
Before quitting the military, all service members have to undergo Transition Assistance. DD Form 2958 is submitted to the command along with other paperwork and is eventually used to prove to Congress that the retiring service member obtained the required training.
The form is used to make sure retirees are acquainted with all the benefits that they have the right to have access to, including VA or Job Center benefits. It also serves to prove that they have been counseled on all of the issues of financial, personal, or family needs.
Before quitting active service, a soldier must meet up with a counselor who is tasked with reviewing their ITP with DD Form 2958 to make sure they meet the requirements of Career Readiness Standards. There are six blocks on the ITP Checklist.
Section I is filled out while undergoing Pre-Separation Counselling Briefings. Sections II to VI are filled out in accordance with the chosen career. Service members must fill out the section covering employment if they wish to enter the job market after finishing their military training.
The Education Block of the ITP is filed if the service member is planning to pursue higher education. An ITP can be modified in accordance with particular requirements. Don't forget All changes in an ITP Checklist must be saved and all entries must be dated. The instruction on how to fill out the Army DD form 2958 are as follows:
In Section I enter your general information: name/family, grade, DoD Identification number, expected date of retirement, specific service branch, and current unit.
Fill out Section II according to your Career Readiness Standards (CRS). A response is mandatory for each entry except for Item 10, only applicable to Active Component Service members and Boxes A to D in Item 16, only relevant to service members pursuing college or vocational education.
Pay close attention to Item 20 in Section IV. You have to select the item from the drop-down list corresponding to the reason you may be exempt from attending a DOL Employment Workshop. The NOT EXEMPT option is to be chosen at all times except for the following cases:
Enter the name and the contact information of the Veterans Administration, Department of Labor, or other employment, educational, or supporting resources available at your final post-transition destination to provide assistance to you after leaving active military service in Section V. A warm handover is required for those who need further assistance. The warm handover is a confirmed person-to-person contact between the retiree and an appropriate partner agency.
Commanders of their designers verify that the service member meets the CRS and has a viable ITP at Capstone in Section VI. If not, they will take action to connect members to the appropriate local resources for the necessary assistance. Commanders will then document the warm handover in Section V.
