How to Fill Out a Navy Special Request Chit?

Navy Special Request Chit instructions are as follows:

  1. Write down your name, rate, ship or station, department or division, and duty section or group.
  2. State the date of the request and describe its nature - leave, special liberty, special pay, or commuted rations. If your request is not listed in the form, you may check the box "other" and explain the request below.
  3. Enter the number of days requested and specify the date and time of the request. If you travel, indicate the mode of travel and its distance. Provide the leave address and telephone number.
  4. Describe the reason for the request in full detail. For example, you may need a leave to attend a Naval Technical Training Center class;
  5. Sign the form.

Once you complete the form, present it to the official in your department or division who is responsible for approving or disapproving your request. The commanding officer will check the appropriate box ("yes" or "no"), write down their rank or title, date, and sign the form.   1. Keep the Navy Special Request Chit in your possession at all times;
