Utah New Hire Registry Instructions

1. Before completing the form you will need to fill out each section in all caps (typed in a size ten or twelve plain font or written in blue or black ink). Forms completed with lower case letters will be rejected and you will need to submit a new form.
2. Include the name and phone number of the person responsible for all new hire paperwork.
3. The date this form was completed (note, a DWS-UI Form 6 must be submitted within twenty days of the first start day for all new hires).
• Any Utah New Hire Form not submitted within this time period is subject to a fine between twenty-five to five hundred dollars.
4. The employer's information, such as name, address, and federal identification number.
5. The new employee's information, including the following:
• Full name (first, last, and middle initial);
• Home address;
• Social security number;
• The first full or partial day of work the employee completed at your business or organization (or the date the employee returned to work at your business if they have not worked there for over sixty days);
• The employee's date of birth, however, this is optional to include.

When you are submitting the form, you will also have the option to include a copy of the new employee's W4 paperwork, however, this is not a required item. Be sure to maintain the original document or a copy for your business records.
