DA Form 3078, Personal Clothing Request , is a record of inspection and inventory of the clothing received by Active Duty Soldiers in the U.S. Army. DA Form 4886, Issue-in-kind - Personal Clothing Record, is used for the same purposes but with Reserve Componentservice members.
The latest version of the form - sometimes incorrectly referred to as DD Form 3078 - was released by the U.S. Department of the Army (DA) on July 1, 2014 . An updated DA Form 3078 fillable version is available for download and online filing below or can be found through the Army Publishing Directorate website.

DA Form 3078 is used for providing accountable documentation of clothing items issued to enlisted personnel. Army Military Clothing Stores (AMCS) must also maintain copies of DA Form 7000, Delegation of Authority - Army Military Clothing Stores, for commissioned officers. The list of main and additional authorized clothing allowances is provided by the Common Table of Allowances (CTA) 50-900.
DA Form 3078 contains detailed reasons for the original issue of clothing and Army Service Uniform (ASU). The form must have orders attached to it. In case the soldier is a member of the Army National Guard (ARNG) or the U.S. Army Reserve (USAR), clothing supply personnel can be contacted via the Kentucky Logistics Operation Center.
Female soldiers are authorized to wear the maternity ASU during the last stages of pregnancy once the regular uniform is no longer appropriate for the condition. Maternity ASU consists of a maternity shirt, a skirt, slacks, and a tunic as listed in the CTA 50-900. These uniforms are maintained for 3 years and are excluded from the supply system.

DA Form 3078 (filled out and signed) is forwarded to the unit commander for approval by unit supply personnel. The original form is kept along with four copies by authorized personnel. One copy is given to the soldier, two copies are retained by the unit (one for future reference and one until the completion of the 180 days of service), and two more are kept by the servicing finance office (a copy and the original form).
