DA Form 5519-R, Tool Sign out Log/Register , is a document used for keeping the records of the toolsets, kits, and outfits (SKO) issued for use within the United States Army. The form is unclassified and approved for public release and distribution: the letter R at the end of the form stands for "reproducible".
The latest edition of the form was released by the U.S. Department of the Army (DA) on April 1, 1986 , and is valid and in use today. An up-to-date fillable DA Form 5519-R is available for digital filing and download below or can be found on the Army Publishing Directorate website.
DA Form 5519-R instructions are not distributed along with the form. All information about filing and usage is provided in the Department of the Army Pamphlet 710-2-1, Using Unit Supply System (Manual Procedures).
All the SKO are stored in the tool room until they are needed. The tool room is under the supervision of the custodian, who is responsible for the safekeeping of the items. The custodian keeps the list of authorized personnel that has the right to sign the equipment and prepares a log for each SKO on hand.
The DA 5519-R Form acts as a log of all issued items and contains information on each: the name of the tool, National Stock Number (NSN), the quantity issued, issue & return date, and the name & signature of the person taking the tool. When the equipment is returned, the tool room custodian indicates this in a document.
The Army tool log should be filled out in ink or completed digitally. To ensure all the tools and supplies are returned, the custodian checks the log at the end of each work shift. After all of the tools indicated in the paper are safely stored within the tool room, the document is destroyed.
