DA Form 3020-R, Magazine Data Card (MDC) , is a form used for recording all information about the munitions issued, received, stored, or held in stock within the United States Army.
The latest edition of the form was released by the U.S. Department of the Army (DA) on August 1, 1989 , and is still valid and in use today. An up-to-date fillable DA Form 3020-R is available for download or digital filing below or can be found on the Army Publishing Directorate website.
The MDC - often confused with DD Form 3020, Breaching Reconnaissance Record - is a non-regulated document and can be printed on demand. The letter "R" following the number of the form stands for "reproducible."
The Magazine Data Card is usually filled out by the checker responsible for handling the ammo in the storage unit. DA Form 3020-R is time-sensitive: the checker has 20 minutes to fill the form out.
The MDC is filled out digitally or handprinted and attached to every lot of ammunition in the storage area. The information in the form should be accurate and legible. The form requires DoD identification codes, lot numbers, the location, description, quantity, and purpose of the items stored. This data must comply with other documents accompanying the listed ammunition. Their identification numbers should be provided in the Magazine Data Card as well. The accuracy of the MCD record is checked by the person in charge of the form.
