A Utah Medical Power of Attorney Form gives a person the opportunity to select a medical representative that will be given rights concerning medical treatment and any relevant information that may be required after death. The agent is only able to make these decisions if a doctor rules that the principal is no longer able to do this independently. Therefore, the agent needs to be trustworthy (which is why agents are often family members or close friends) and should be available at short notice. Local state laws in Utah stipulate that in order to form a legal Utah Medical Power of Attorney, only one individual of legal age should be present as a witness. This witness must be "disinterested" in regards to the principal i.e. not expecting anything from the will of the person.
