A Missouri Medical Power of Attorney Form is a legal document granting another individual permission to make important medical decisions (those usually concerning treatment) on their behalf. These decisions can be life changing, hence why the principal must carefully think about which individual they can trust to become their agent - someone that is reliable and someone that will still put the ideas of the principal in first place. The chosen individual can only use these powers once the principal cannot communicate anymore and this information has to come from the doctor. This will greatly put the mind of the principal at ease and if they have specific concerns or things that are extremely important to them, they must share this information with the agent.
Before sitting down to draft and sign the Missouri Medical Power of Attorney, you will need to sign this document in front of a notary public. Additionally, if any specific declarations are made then they must do this in front of two witnesses as well.
