A Hawaii Medical Power of Attorney Form gives permission for an agent to represent a principal in a medical setting, allowing them to make specific choices relating to treatment and any other significant medical decisions. The agent may only do this if the principal is deemed as unfit to do so themselves which usually occurs if they are found to be in a coma or are experiencing neurodegenerative diseases that could affect their clear thinking. The agent should act in the best interests of the principal and fulfill any wishes that they had regarding treatment. This is why the principal will often select a close friend or family member, someone they can be certain of and trust.
A Hawaii Medical Power of Attorney should be signed appropriately. Failing to do so could result in questions over the legitimacy of the document being raised. In Hawaii, this document should be signed by a notary public or by two witnesses. The criteria for those that qualify as witnesses are outlined by the state law.
