An Alaska Medical Power of Attorney Form is created when a principal wants to delegate themselves an agent with the legal rights to make certain medical decisions in their favor, on their behalf. This is usually applied when the principal is no longer in a fit state to make decisions for themselves independently. The principal has the right to choose a second agent if they desire to do so. Once the Alaska Medical Power of Attorney is signed (taking into account the local requirements concerning the signing of the document) it should be stored away in an appropriate place where it can easily be retrieved should there be such a requirement.
Local state regulations in Alaska are very specific about their signing requirements. Apart from the principal and the agent(s), the document must also be signed by and in the presence of either a notary public or two witnesses. Most people in Alaska will opt for using a notary public because there are stringent criteria concerning who is allowed to act as a witness.
