All parties involved in signing such documents need to confirm that they are doing so freely and if the notary public is happy with the statements made, they will confirm the identity of each party. After this, the individuals must sign the document in question but this does not necessarily need to be done for the notary public to witness - however, this could still be a wise idea to avoid any future misunderstandings.
