What Is CAPF 160 Form?

Form CAPF 160, CAP Member Health History Form , is a document filled out by cadets prior to entering the Civil Air Patrol (CAP) program. A CAPF 160 fillable version was released on June 1, 2013 , and is available for download below.

The purpose of the form is to collect the medical history of cadets and to provide the consent of a parent or a legal guardian for minor cadet participation in CAP programs as well as for medication and treatment. The medical history indicated in the form includes various types of information: from the vaccines the cadet has had to any current medication they may be taking. Due to the amount of personal medical information listed on Form CAPF 160, it is confidential and for official use only.

Form CAPF 160 Instructions

According to CAP Regulation 160-1, Operation of the CAP Health Service Program, issued by National Headquarters Civil Air Patrol, a copy of the cadets' CAPF 160 should be kept on hand when participating in a CAP activity. The filled-out form contains a lot of important information concerning the cadet's health that might be helpful during an emergency situation. Types of vital medical information the form includes the following:

  1. Allergies. All cadets are required to list any allergies they have (besides food allergies), the medications they take for these allergies, and the types of reactions they have.
  2. Dietary Restrictions or Limitations. In this section, the cadet must designate whether they have diabetes, any food allergies, or if they follow a gluten-free or vegetarian diet.
  3. Past Surgical History. This section requires a list of surgeries the cadet has had (including an appendix removal, hysterectomy, etc.).
  4. Immunizations. In this section, the cadet must list all vaccines they had (i.e., Hepatitis, Influenza, Tetanus, etc.).
  5. Medication Information. The cadet must indicate what medications they will need to take during CAP activities.
  6. Social History. The cadet must describe their occupation, religious preferences, and smoking history.

In addition to the items listed above, Form CAPF 160 includes a long list of various conditions that may affect the activities during the program or encampment, from which an applicant must check the ones they have. An applicant is also required to give explanations or mention any other conditions they have in the remarks section. If the space provided in the "Remarks" section si not enough, the cadet is allowed to attach additional sheets to the form in order to include all relevant information.

The last part of the form ("Consent for Minor Cadet Participation, Medications, Treatment") includes three statements:

  • Permission to participate in a CAP program;
  • Permission for minor cadets to possess prescription medications listed above;
  • Permission to secure proper treatment for minor cadets.

All of the permissions must be signed by a parent or legal guardian of a minor cadet. Otherwise, without permission, as a general rule minor cadets must not bring any prescription or non-prescription vitamins, medication, herbals, or supplements to any CAP activity.
