Here is how you should complete the HUD Supplement to Application:

1. Indicate your full name, mailing address, and telephone numbers.
2. State the name of the individual that can be contacted, their address, and telephone numbers. Alternatively, you may write down the name of the organization you have been working with or the one that represents your interests. You may add the e-mail address of the person or entity to contact if this is the preferable way of communication - if, for instance, you are late with your rent payments, you will receive a written notice about your debts, and then your contact person will receive a similar letter that proves you were unavailable during your tenancy.
3. Record your relationship to the contact person or organization - there is no need to attach documents that confirm your relationship.
4. Choose the reason for contact - your family member, friend, or attorney may be contacted if there is an emergency, you do not answer the calls and letters, you are about to be evicted or you failed to make a rent payment, or to inform you about any changes in the lease or house rules.
5. If you prefer not to complete this form, check the appropriate box to certify your refusal to provide contact details.
6. Sign and date the form.
