To make sure this document is enforceable, you need to write down the date of signing, indicate the names of the landlord and tenant, state the legal address of the rented premises, confirm the readiness of the parties to transform the original lease into a month-to-month lease, record the right of both parties to terminate the agreement with the help of a written notice, enter the new rent amount if it is increased or decreased, and sign the document - or ask your legal representatives to do that for you. While the notarization is not mandatory, if the lease contains a notary seal, consider notarizing the addendum as well. Each party must have a copy of the addendum that has to be kept with the copy of the lease agreement.
