What Is a Waxing Consent Form?

A Waxing Consent Form is a document filled in by a client that agrees to have some form of wax procedure done to them. Although the procedure is safe and does not carry any risks, it is still best to sign such a form, as it protects the customer and the individual carrying out the procedure from any legal problems, should any issues arise. The trained professional will usually brief the client about the process of waxing and explain the benefits.

A simple Waxing Consent Form usually contains details regarding the customer including their full name, date of birth and full address. There is also a section asking the client to state any known health conditions that they are aware of. The details of the procedure, particularly the pros and cons should be clearly outlined in the consent form. It will also detail the conditions and policies that the individual or company has in place for the procedure.

At the end of the form, there is space for the client to print their name, signature, and date, which acts as confirmation that they agree to the procedure and are fully aware of the conditions listed in the document.
