A Field Trip Consent Form is given to the legal guardian of a child, usually the parents, for them to fill in an agreement to take their child on a field trip - in most cases these are handed out by schools. The parents must also inform the individuals accompanying their child about any known relevant medical conditions or medicine that the child would need to take. This would also be extremely useful in an emergency situation if the child has to be taken to a doctor. The accompanying adults are solely responsible for the medical aspect of the field trip.
