Paternity Acknowledgment Form

A Paternity Acknowledgment Form is used by unmarried parents to officially legalize the father and establish this in the child's birth certificate where they will be listed as the biological father.

• Acknowledgment of Parentage Form.

This is providing that both parents are in agreement, they do so voluntarily and there is no ambiguity as to who the father may be. This procedure simplifies the process a whole lot more, rather than taking it to court. Signing this form will provide the same legal rights that a court would establish. You cannot sign the form during pregnancy, only after the birth of the child.

There are several advantages to signing such a form:

• A record is kept of the parents along with relevant medical history that could aid in treatment;
• The father gains the right to custody or visitation orders;
• If adoption is proposed, a father will have an input into this process;
• Ensuring parental responsibilities are shared;
