What Is a Video Consent Form?
Have you ever been to an event where you signed a Video Consent Form ? If you have and you signed it, you gave the recipient permission to use your likeness in one or more forms of multimedia.

- Video Consent and Release Form;
- Video Recording Consent Form.
It is common for Video Consent Forms to include a clause that informs the participant that they will not receive payment for their consent to be photographed or recorded. Even if you happen to end up on a popular billboard or viral commercial, your beautiful face is stuck there. When you signed the Video Consent Form you legally gave permission for a third party to use your face and/or voice. Legal action may be futile as a well written consent form includes an agreement that the participant will not take legal action. If you are signing a Video Consent Form, be sure you thoroughly read and understand all information within the document. If you are distributing a Video Consent Form, be sure to cover all your bases to avoid legal litigation.

How to Write a Video Consent Form?
If you are considering taking photos of or recording other individuals, it is important that you know the components of a basic Video Consent Form. If only taking photos, be sure to check out our Photo Consent Form template. There are five main components of a basic Video Consent Form.

If you are an individual, organization, or an individual operating on behalf of an organization, be sure to make that clear in your Video Consent Form.
- If you do not state your name or organization's name then it is not clear who the participant is giving consent to.

Name, signature, and date the participant is giving permission for you to collect, publish, and reproduce their name, voice, and likeness in different forms of media.
- These forms of media are up to you and should be specified in your Video Recording Consent Form;
- If you want to be thorough, you can also collect the address of the participant.

A detailed informed consent clause.
- This is a clear explanation of what it means for the participant to grant permission to you or a third party to use them in your content, what your content is used for, and any possible future uses.

It is also important to mention whether or not individuals will receive compensation for their participation.
- If you do not intend to compensate participants, be sure to include a clause stating that participants waive their rights to receive royalties.

For the sake of clarity, you may also want to restate your rights and the participant's rights. If you have any legal information to include, it is best practice that you do.
- You want to protect yourself from the possibility of an individual taking legal action against you.

In the case that the participant is a minor, you are going to need the name and signature of their legal guardian.

In the case that the form had to be read to the participant or their legal guardian, the interpreter must also date and sign the Video Consent and Release Form.
