What Is an Invisalign Consent Form?

An Invisalign Consent Form should be signed by a patient if they consent to using the Invisalign treatment plan. It is becoming a much more popular way to straighten out teeth compared to wearing braces - mainly because the aligners are clear and they are not as visible as braces.

The purpose of the consent form is to provide the patient with information regarding the treatment plan, any potential risks involved and the responsibilities of the patient. By outlining this information, the patient agrees to take responsibility for the treatment and this will protect the orthodontist and Invisalign company from any potential lawsuits from the side of the patient.

Specific information about the treatment, particularly information about any potential risks should be clearly shown in the consent form. If the patient agrees to all of the conditions and is happy to proceed, they must print their name, sign and date the consent form before handing it over to a medical representative. In some cases, a witness may also be required to sign the consent form.
