Standard Form 1408 Instructions
To complete this form you will need to fill out the following sections:
- The Recommendation section will confirm that the accounting system which the contractor plans to use is in accordance with the rules established by the government review committee.
- If your company does not meet the requirements or will in the future you will need to explain why in detail.
- The Evaluation Checklist will ask the contractor to complete a series of yes or no questions about how the accounting is performed (such as keeping direct and indirect costs separated at all times).
- The contractor must also agree to allow for a review of their financial system and confirm they are fully operational to meet the accounting demands that the government department will require from them.
