What Is a Tattoo Consent Form?

A Tattoo Consent Form is a formal document prepared by the tattoo salon and signed by the client who confirms they agree to receive a tattoo. Every client of a tattoo parlor must be offered this document before the tattoo master starts their work - it will confirm the person agreed to get a tattoo and absolve the artist of any responsibility for complications that may arise later provided that the client is fully informed of all possible health and safety-related issues.

The document allows the person who wants to have a tattoo to be fully educated about the procedure they will undergo - they can ask for clarifications, modify the agreement if necessary, and get a tattoo with peace of mind. The tattoo master will be protected as well - they are not obliged to proceed with negotiations or start their work if the individual tells them the procedure may be harmful to their health.

How to Write a Consent Form for a Tattoo?

  1. Identify the client and the tattoo studio by their names. Since the client usually chooses one master to work on their tattoo and sticks to their choice, make sure you include the name of the artist as well.
  2. Describe the tattoo in question. Whether the client wants a simple design or it is a large tattoo that requires days or weeks of work, it may be a good idea to attach a drawing the parties have agreed upon to avoid possible disputes in the future.
  3. List the risks of the tattoo procedure - make sure you write "including but not limited to" to safeguard yourself from the liability for any injury and bodily harm the client may have in the future. At a minimum, you should mention scarring, infection, allergic reactions, and bleeding.
  4. State the client was asked about any health issues that may prevent them from getting a tattoo and their answer was negative.
  5. Certify the client has been fully informed of all the risks associated with getting a tattoo and confirmed their willingness to receive it.
  6. If the client is a minor, the tattoo master must get parental consent - one or both parents have to sign a Tattoo Consent Form for Minors. Not every state allows a minor to be tattooed so ask your tattoo parlor whether they are able to tattoo a minor accompanied by a parent or legal guardian who will sign the Minor Tattoo Consent Form and will be present during the procedure to make sure the child is safe.
  7. If you are a laser technician that specializes in tattoo removal, you can use a Tattoo Removal Consent Form - this document will verify the permission of the individual prior to any procedures and treatment. List short- and long-term side effects and complications of the laser removal, record aftercare guidelines the client has to follow to heal and prevent scarring and obtain their authorization for the procedure.
