To create a Change Order Form include the following information:
Name of the construction project.
Location of the business or work site.
The contact information for all construction managers, owner, and lead engineers.
Name of the party requesting the change and date.
Identification numbers for the contract and change order.
Create a table listing the changes that need to be made, the reason for the changes, any supporting documentation, and specifications for each change.
Create a second table listing all items and materials that need to be purchased to complete the new requirements.
A final table with the name and contact information for the person(s) requesting the updates.
Signatures for all project heads.
