A Player Evaluation Form is a written document used by coaches, team managers, scouts, and agents to examine the player from different angles, provide necessary feedback so that the player is able to improve their talents, and decide on promoting the person being evaluated to another level. It is possible to customize the document to suit your needs, but at a minimum, an evaluation should identify the player and the individual behind the evaluation process, assess the player's psychological state and technical skills, list areas they can succeed most whether it is a defending or an attacking role in the team and include scores you give to every player you have observed.
