How to Write a Course Evaluation Form?
Follow these steps to draft a Course Evaluation Form:
- Indicate the number of the course and the name of the professor or instructor responsible for teaching it.
- Although an anonymous evaluation is more common, you may add a line for the student to write their name in if they are willing to share it with the person who will review the forms. If the student expects feedback, ask them to check the appropriate box and state their contact information so that they are able to receive an answer.
- Ask the student whether the course they are assessing was required or optional for them - often, an elective course becomes more popular because students share positive feedback, and eventually, it will allow the school or college to make this course mandatory for all students.
- List different criteria that show the effectiveness of the course. Depending on the discipline, you can include as many characteristics as necessary. At a minimum, ask students whether the manuals and supplemental materials recommended or required by the school proved to be useful, were these textbooks used regularly or the professor only referred to them a couple of times during the course, and how the description of the course reflected the content students were offered.
- Add questions about the instructor - find out whether they were well-qualified to teach this particular course and how was the communication and rapport between them and students. You can add blank space for the student to record any incidents or issues related to the instructor's behavior.
- Leave blank space - half of the page should be enough - for the student to enter any new ideas and suggestions. You can give them an example - for instance, the student may think another textbook would be more appropriate for the course or the attendance can be improved if the frequency of lectures or lessons is lower.
