Form AW8, NHS Pensions - Retirement Benefits Claim Form , is a document used by individuals to claim retirement benefits from the United Kingdom National Health Service (NHS). If the individual in question is no longer employed by the NHS, they are supposed to use another document: Form AW8P, Deferred Benefit Claim Form.
The AW8 Form is divided into two parts. The first part is supposed to be completed by the employer of the applying individual. The second one must be completed by the individual themselves. According to the instructions, the individual must complete the second part and return it to their employer three months before their retirement. After the individual has filled out their sections and returned the form, their employer is supposed to complete the first part of the form. It includes information about their pension scheme membership, pensionable pay details, verification of information provided by the applying individual, etc. After the employer has completed their sections of the claim, they are supposed to process it on Pensions Online (POL) - an online tool that can be found on the official website of the NHS in the "Employer Hub" section - or send it to the NHS Pensions.
