VAT Exemption Form Instructions

The VAT Exemption Form is prepared by the supplier and the customer. The information required from the supplier includes the following:

1. The full name of the individual representing the business in question.
2. The name and address of the business.
3. A confirmation of the fact that the goods or services are being supplied to the individual named on the second page of the declaration.
4. The category and a brief description of the goods or services in question. For instance, you may be responsible for repair services to the customer's residence - in this case, you can indicate the location of the works and the name of the individual who will carry out the repairs.

The customer is required to complete the following steps when filing the VAT Exemption Form:

1. Read the guidelines on the second page to make sure you are eligible for the VAT relief. Note that the form can be prepared by a relative or carer if you are unable to do so for some reason.
2. State your full name and address.
3. Confirm you have a disability or chronic illness and specify your medical condition in the provided space.
4. Certify you are receiving goods or services listed in the form for your personal or domestic use and you want to claim VAT relief.
5. Sign and date the document. Give the completed VAT Exemption Form to the supplier for their records.
