How to Fill Out DVLA Medical Form?

  1. Start with your personal details (include your title, full name, and birthdate). Share your contact information (your address, telephone numbers, and email address). If you have a driving license number, write it down.
  2. Indicate the names and contact details of your general practitioner and consultant. State the title and department of the consultant. Record the dates you last underwent a medical check-up with these specialists regarding the condition you specify in the form.
  3. If you are attending other medical institutions, list them and enter the reasons for all visits.
  4. Write your last name, birth date, driving license number, or a reference number you have received when applying for a license at the bottom of each page.
  5. Fill out the questionnaire to describe your vision. Tick the box that correctly states your current vision condition. If there are multiple issues, you are allowed to select several boxes.
  6. Tick the box to specify how many functioning eyes you have and which eye has been affected by the medical condition.
  7. Answer "yes" if you have even sought laser surgery or injections to treat the eye condition you have and specify whether you have informed the DVLA about this treatment.
  8. Tick the box to certify your doctor has told you about a problem you have with a field of vision and confirm this problem was caused by the eye condition. If this issue was triggered by a head injury, brain tumor, or stroke, state it as well.
  9. If you are suffering from double vision, tell the DVLA how you are controlling it and whether you have visited an eye specialist to obtain consultation regarding this issue.
  10. Carefully read the regulations regarding glasses, lenses, a patch, or prism you have to wear because of your double vision, and agree not to drive before proper medical advice from your doctor.
  11. Examine the current eyesight standard for driving, make sure you comply with the relevant requirements, and tick the box to show whether you can properly see with or without low vision aids.
  12. Confirm the information in the form is true and correct, allow the DVLA to inform your doctor about the outcome of the declaration, and agree to the release of your medical information to your doctor. Add your name, sign and date the form. If you want to be contacted via text message or email, answer "yes."
