Sponsorship Undertaking Form Instructions

Follow these steps to complete the SU07/12 Form:

1. Enter the full name of the sponsored individual and underline their surname.
2. Record the date and place of birth of the applicant and state their nationality.
3. Write down the full address of the applicant to inform the government where they will be living in the United Kingdom.
4. Indicate the full name of the sponsor and underline their last name.
5. Enter the sponsor's date of birth and National Insurance Number.
6. Add the full address of the sponsor. Note that you must attach a proof of address to the form - for instance, you can enclose confirmation of mortgage or a utility bill to show the sponsor actually lives at the address provided in the form.
7. Identify the employer of the sponsor by the name of the company and its full address.
8. Obtain the signature of the sponsor. They must confirm their willingness to help the person named in the application to settle in the United Kingdom - they will bear responsibility for the applicant's accommodation and maintenance and they certify they will not need any public funds to do so. Finish the document by writing down the actual date of filing.
