Form S1 Instructions

1. Personal Information. In the first part of the document, the individual must provide their personal details, contact details, information about their spouse (if applicable). Individual's personal details must include their full name, date of birth, place, and country of birth. Contact information must include their present address, daytime telephone number, mobile telephone number, and email address. If the individual's marital status is "married" then they are required to provide their spouse's name, date of birth, place of birth, country of birth, and nationality.
2. Residence Requirements. Here the individual can designate details of the parent on whom the applicant's qualification depends. It includes the parent's name, date of birth, place of birth, and other details.
3. Referees and Identity. When the individual is completing this section of the form, they should start with affixing their photograph in the designated area on the page. Once they have done it, this part must be filled out by their referees. Here individuals can also find requirements for the referees (age, citizenship, occupation, etc.). This section is divided into two parts, each one of them is supposed to be filled in by different referees. They must state their name, age, profession, contact details, how they know the applying individual, etc. At the end of this part, the referee is required to put their signature and date.
4. Date and Signature. The last part of the document contains several statements that the individual must acknowledge and agree on. The statements designate that the individual has paid all of their required fees, enclosed the appropriate evidence, provided true and correct information, and agreed to disclose information. To express their consent, the individual must sign and date the document.

After the individual has completed Form S1, they are supposed to gather documents that must be attached to their application. The full list of appropriate documents will depend on the circumstances of the individual's situation, they can include different letters from the authorities that confirm the facts that the individual has stated in this form, various certificates, letters from employers, schools, and so on.
