PIP2 Claim Form Instructions

Form PIP2 requires the following information:

1. Information About the Individual. In the first part of the document, individuals must designate their name and national insurance number.
2. Information About the Health Professional. Here individuals are supposed to provide information about hospital doctors, social workers, physiotherapists, and other health professionals who can advise the Department for Work and Pensions (DWP) on how the individual's health condition affects their life. The information must include the professional's name, address, profession, phone number, and other details. If the DWP needs any additional information on the individual's health condition or disability they will contact the health professionals listed in this part.
3. Information About the Health Condition or Disability. Individuals must use this part of the form to designate which exact health conditions or disabilities they have and the date when they have started (approximate). In addition to this, they must indicate any medication they are taking and its dosage, if it has any sides effects, whether they are having any treatment, etc.

The PIP2 Form contains other sections as well, which are supposed to explain better how the individual's health condition affects their life. In these sections, the individual is required to provide a description and examples of how their health condition affects their daily life. It includes details on whether the individual experiences any difficulties while eating or drinking, if they use any appliances while bathing or showering, if they need any kind of help from other people while dressing, and other examples.
