Form N161 Instructions

Here is how you need to complete Form N161:

1. Record the details of the case or claim you are filing an appeal against (its number, names of the parties, personal details of the appellant, and potential respondent).
2. List the particulars of the appeal (provide the court, the name and status of the judge, and the date of the original decision).
3. If you have a legal representative or know about the respondent's representation, name the solicitor or counsel that will handle the appeal on your and the respondent's behalf.
4. In case you require permission to submit an appeal, request it or confirm you have already obtained the permission.
5. Describe what order or part of it you are contesting. State the grounds of appeal and arguments to support them. Enclose them with the notice.
6. If your matter relates to environmental justice, tick the appropriate box and enter the grounds in the provided space.
7. Indicate what outcome you are looking for - to get a new trial or to cancel or modify the existing order.
8. In case you are going to make supplemental applications, specify them and provide evidence to support your claims.
9. Confirm all the statements in the notice are true and complete, sign and date the form. List documents attached to the notice.
