HC1 Form Instructions

Follow these steps to prepare the NHS HC1 Form:

1. Provide your personal information (your full name, title, birthdate, National Insurance Number, telephone number, and address). Add the same details for your partner. Tick the boxes to indicate health expenses you want to be compensated.
2. List your dependents whose education you financially support (write down their names, dates of birth, and their relationship to you).
3. If there are other individuals who live with you, name them, state their relationship to you, confirm their tenancy if applicable, and tick the boxes to show their sources of income.
4. Describe your assets. These include money from bank accounts, bonds, shares, investments, savings, and the property you or your partner own, including foreign real estate.
5. List all types of income you receive every week or month. You have to indicate pensions, social security benefits, maintenance payments, allowances, credits, Armed Forces payments you and your partner are entitled to.
6. Describe your and your partner's employment situation do you work, how many hours you usually work every week, how often, and how much are you paid.
7. Outline your living situation - are you currently in the medical facility, do you live with your family members or friends, do you own your residence, and do you rent the place where you are residing at the moment. Provide the breakdown of your rent payment and additional charges you deal with.
8. If you or your partner receive education, fill out the HC1 Form for students to certify you are enrolled in a school or university and you wish to request financial assistance with your health costs. Record the name of the educational institution, details of the current academic year, and the type of your qualification. List sources of support you have applied for to reduce the costs of education - alternatively, you may claim the money is coming from your work or relatives. Indicate your residence during the academic term.
9. Use free space to add more details the NHS should know about.

Where to Send Form HC1?

Once Form HC1 is completed, mail it to the Low Income Scheme (NHS Business Services Authority, Bridge House, 152 Pilgrim Street, Newcastle upon Tyne, NE1 6SN). You will receive help if your income does not exceed your living requirements or is up to half of your current prescription expenses. The NHS will provide you with a certificate to receive assistance within four weeks of submitting the claim.
