How to Complete ET1 Form?

  1. Claimant's Details. In the first part of the document, the claimant is required to provide their name, date of birth, and contact information. In addition to this, they must indicate how they would prefer to be contacted and whether they will be able to take part in a hearing by video (this requires internet access).
  2. Respondent's Details. Here the claimant must give the name of the person or organization they are claiming against, their address, and phone number. If there is more than one respondent, the claimant is supposed to provide information about all of them.

  5. Earnings and Benefits. Claimants can use this part of the document to designate how many hours they worked each week in the job this claim is about. They must also indicate how much they make (used to make) if their period of notice was paid (if applicable), whether they were included in their employer's pension scheme, and other details.
  6. Type and Details of Claim. Here the claimant is required to state the type of claim they are submitting. They are offered to choose from a list, which includes types such as unfair dismission, discrimination, redundancy payment, being owed payments, etc.
