Mandatory Reconsideration Form Instructions

You will want to include the following on the CRMR1 Form:

1. General information about the person this request is in regards to (their name, date of birth, National Insurance Number, the benefit in question, address, telephone number, time of day that is best to contact the person submitting the request). If you are completing the form on behalf of an individual who cannot complete it themselves (such as a child, elderly person, or adult who requires assistance), fill in the section as their representative assisting with this process. For this portion, you will include your general information along with your relationship to the person you are representing (parent, carer, guardian, legal representative).
2. Form CRMR1 will ask if you are asking for the decision to be reviewed again within a one-month period or if you will need additional time and why.
3. The next section will give you the opportunity to explain in full why you are disagreeing with the decision. You will need to explain which parts you do not agree with and include any new information that would help support your claim. All additional evidence will need to be included with the form and each document listed on Form CRMR1. If you have evidence or documentation that you think will support your case but are unable to submit it along with the form you will also need to explain why. For example, if there is a medical report you are waiting to receive but it will be unavailable to you by the time you need to submit this form you are able to include such a statement.
