Form 4297-82, Health Card Renewal , is used when you are nearing the expiry date on your current health card and will need to update your information. This is the document you will need to complete before visiting the nearest Service Ontario center.

Just like with a driver's license, the Ontario Health Card Renewal Form will have a picture of yourself on the card to verify identification, and as you age, your card will have an expiration date to ensure that the photo on the card is up to date. Once a person has reached age fifteen and a half, they will be required to update their Ontario Health Card in person and have a photo taken at the service center.
If you are older than eighty years of age you will also be given the option to mail in your renewal form and will receive your renewal notice by mail. Before completing the Ontario Health Card Renewal Form you will want to make sure you are completing a separate form for each person who will need to renew their health card.
