DD Form 2768 Instructions

Follow these steps to fill out Form DD 2768:

1. Enter the departure and arrival International Civil Aviation Organization (ICAO) codes for your airfield.
2. Indicate the earliest departure time and expected arrival time in Greenwich Mean Time (GMT).
3. State the purpose of travel and compelling operational considerations, record the total number of passengers and cargo weight.
4. Describe the costs incurred between the origin of travel and destination for the leg.
5. Identify the preferred request coordinator, departure coordinator, and arrival coordinator by their names, grades or ranks, duty descriptions, and contact details.
6. Name the senior traveler and additional travelers.
7. Describe the cargo, add special handling instructions, and item dimensions and weight.
8. Write down additional comments or remarks in the provided space.
9. Obtain signatures of the senior traveler and authorizing official. They must also state their duty title, telephone numbers, and date the request.

Once you complete the request, present it to the validation office - a designated official will assign priority and necessary codes, and validate your document. It will be kept in the validation office records for three years from the day of validation.
