A standard Video Release Form is a document that grants permission to a third party to utilize the voice and appearance of an individual in both videos and still images for the advantage of a said third party. Such advantages can include using the materials for media, online, physical products (such as DVDs or CDs), or any other publication. Once the individual signs the document, they release the rights to their image, video, still, and voice to the third party. A Model Release Form is different from a Video Release Form or Photo Release Form in that the former is signed by the talent with the understanding that the photographer will be able to license or use the image(s). Both Photo and Video Release Forms are signed by the photographer and returned to the client so the client can use the images or material (i.e. wedding photos, headshots, etc.).
A Video Release Form covers privacy laws, permission from the client, and obtaining signature(s) on the document. Privacy laws (both state and federal) will require consent to use other people's likeness in photographs or videos. This document is a legally binding document and should, therefore, include all identifying information for the client, such as name, address, contact information, a section for the signer to verify that they are eighteen (18) years of age or older, and the date the agreement was signed. A Video Release Form for minors will also require the signature of a parent or guardian. Since videos are considered a compilation of multiple photos, a Photo Release Form is not needed if a Video Release Form is completed. A music Video Release Form will also need to state the title of the music video in addition to the publisher.
The form must also include a section about what the agreed-upon payment will be by the production company that is to be checked by the subject completing the form.
