Form 4057, Change of Address Name, Date of Birth, and/ or Social Security Number, is a one-page form to change your personal information with the South Carolina Department of Motor Vehicles. It is required to submit this SC DMV Form 4057 within 10 days of any address, name, or other change such as the following. This form is required if you moved permanently from your previously registered South Carolina address, or your licensee name has changed, such as through marriage.
Additionally, this form is used to register any date of birth or social security number corrections. Accordingly, you must have previously completed Form 447-NC, Application for Beginner's Permit, Driver's License, or Identification Card. Information received after completing and submitting Form 447-NC, such as your South Carolina driver's license number, license plate number, and customer number (as found on your registration) are needed to complete this SC Form 4057.

Your signature of this document is also required. As your new address may be close to county lines or a new subdivision, the DMV must take these additional steps to validate your new address.
