Form DS-71, Affidavit of Identifying Witness , is a document issued by the U.S. Department of State (DS) for individuals who are applying for a U.S. passport and who are unable to establish their identity to the satisfaction of a person authorized to accept passport applications. When including this form with a passport application, it is still necessary for a filer to be able to provide other identification of their own.

The DS-71 Form, unlike other similar forms, is supposed to be filled out and signed by a witness, not by the U.S. passport applicant. The document consists of three parts:
After the application is completed, the witness must sign it in the presence of an acceptance agent and provide a current photo identification of their own (for example, driver's license).
