Form INF 1125, Request for Own Driver License or Vehicle Registration Record, is an official form to request your own DL (Driver License) history or vehicle registration records.
This form is released by the California Department of Motor Vehicles (DMV) and was last revised in July 2018.
To get an official copy of your 10-year driving record, fill out Form INF 1125, Request For Own Driver License Or Identification Card (DL/ID), and submit it to the DMV headquarters.
Make sure to include your full name as it appears on your license, DL number with the date of birth, and full address.

Generally, Form DMV INF 1125 is requested to get driving record information for insurance purposes. It is used by drivers to request a personal driving record or vehicle/vessel information. An MVR (Motor Vehicle Record) is a report from your state's DMV (Department of Motor Vehicles). This report includes your DL status and any points or violations if applicable. There's a $5 fee for each record requested. If a driving record or vehicle/vessel record needs to be certified as a true copy on file with DMV; a request at the top of the form for a "certified driver' record" has to be made. This option is free.
Although DL records are public, it is not advised to use this form unless you are requesting your own current information on file with the DMV (Department of Moving Vehicles). It is a common practice that insurance agencies and some employers may request a record for a third party (new insurers or a new employee's records).
