OPM Form 1203-FX Instructions

This form consists of six pages. An applicant has to enter their Social Security and vacancy identification number on each page. The form contains a cover page as well which should not be included when submitting.

1. The title of the job should be indicated.
2. This part contains individual information of the applicant, including their name, mailing address, and phone number.
3. The applicant has to print their email.
4. Information about the filer's place of employment should be completed, including their work address and work phone number.
5. The applicant should answer the questions regarding their employment availability. Indicate "Y" for "Yes" or "N" for "No."
6. The filer has to provide their citizenship information.
7. The applicant should answer the questions regarding their background information. These questions have to be found in the vacancy announcement.
8. Other information. The gender and the date of birth have to be entered.
9. Fill out the information about languages, the lowest grade, miscellaneous information, special knowledge, and test location.
10. The applicant has to provide their claim for the Veteran's preference.
11. Enter the dates of active duty - military service. These dates are required if the filer has claimed Veterans' Preference only.
12. The date the applicant will be available for employment has to be entered. Enter the service computation date and other dates as well.
13. Job preference should be chosen from the list provided in the vacancy announcement.
14. Occupational specialties. The applicant has to indicate an occupational specialty code for their position. It is allowed to apply for more than one specialty.
15. Geographic availability. The filer should choose a geographic location applicable for the employment, and enter its geographic location code.
16. If the applicant is requesting consideration for a Transition Assistance Plan, it should be indicated in this part. To receive such consideration the filer should submit supporting documentation.
17. Job-related experience and personal background information should be provided.
18. The applicant has to answer occupational questions on the last two pages of the form. These questions should be found in the vacancy announcement.
