Form SSA-24, Application for Survivors Benefits , is a document filled out by survivors of the veteran - their surviving spouse, surviving divorced spouse, children, or parents - to claim Social Security benefits.

The information on this form is collected by the Social Security Administration (SSA) to determine whether the survivors of the veteran qualify for these benefits. Survivors Benefits will help you pay for everyday living expenses and medical bills without making a regular income after the death of the veteran you were financially dependent on. The amount of benefits will depend on the number of children you care for, your disability, and age.

Form SSA-24 Instructions
Provide the following information in the Survivors Benefits Application:
- Write down the full name of the veteran and their date of death. Indicate the veteran's social security number, date, and place of birth, name of the father, maiden name of the mother. If the veteran worked in the railroad industry after 1936, answer "yes" by checking the appropriate box.
- State the date the veteran entered active military service and separated from it. Enter the military service number and veteran's grade, rank, rating, organization, and branch of service.
- Explain your relationship to the veteran whether you are a spouse, divorced spouse, child, or parent. Record your date of birth. Enter the Veterans Affairs (VA) file number of the veteran.
- Name the surviving children or dependent grandchildren of the veteran who were unmarried and under 18, 18 to 19, and attending secondary school, or disabled at any time since the death of the veteran.
- Confirm the information in the application is true and complete to the best of your knowledge, enter the date of signing, print your name, and sign the document. Add your mailing address and telephone number to receive documentation from the SSA and VA.
Once you fill out the application, bring or mail it to the regional VA office with one of these documents:
- VA Form 21-534, Application for Dependency and Indemnity Compensation, Death Pension and Accrued Benefits by a Surviving Spouse or Child (Including Death Compensation if Applicable);
- VA Form 21P-535, Application for Dependency and Indemnity Compensation by Parent(s).
Prepare the following documents to support the statements made in the application:
- Proof of death and military service;
- Birth certificate or marriage certificate;
- Divorce decree;
- Proof of citizenship or immigration status;
- Proof of adoption;
- Proof of disability.
